Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. The doctor will review your medical history and answer questions you might have about a planned procedure and anesthesia.
We ask you to provide the following information at the time of your consultation:
1. A referral slip and x-rays from your referring dentist or physician.
a. If your dentist or physician has taken x-rays you may request that they forward them to our office prior to the appointment.
b. Digital x-rays can be sent by your referring dentist or physician through our secure website or our secure e-mail portal
c. If your x-rays cannot be sent by email and there is not enough time before the appointment to have them mailed, pick them up and bring them to your appointment.
d. Should additional x-rays be necessary they can be taken at our facility.
2. A CURRENT list of all medications you are presently taking. Not bringing a current medication list will prolong your consultation appointment.
3. Current medical and dental insurance cards.
4. A photo-containing driver’s license or state issued ID.
a. A utility bill or other correspondence showing current place of residence is necessary if the photo ID does not show the patient’s current address.
b. If the patient is a minor the patient’s parent or guardian is responsible for providing this information.
IMPORTANT: All patients under the age of 18 (19 if you live in Nebraska) must be accompanied by a parent, guardian, or power of attorney at the consultation visit. Adult patients who have a legal guardian or power of attorney must be accompanied by that person to the consultation visit.
